Almost every concrete failure we are called to traces back to what happened before the pour. Base not compacted in layers, no reinforcement, joints cut too late or not at all, or the mix watered down on site to make it easier to place. Concrete is unforgiving about all four. You cannot see any of it once the surface is finished, which is exactly why cheap concrete work looks fine for two years and then fails.
